We are in-network with: Aetna, Cigna, McClaren Health Plan, Priority Health, and United Healthcare. Our office can verify your benefits to find out how much you will pay out-of-pocket for each visit, as well as the total number of visits your insurance will authorize.

We are currently in the process of adding more contracts with insurance companies, including Blue Cross/Blue Shield and companies administering Medicare Advantage plans, Medicare Supplemental plans, and Medicaid plans. Our website will continue to be updated in the coming weeks.

We also offer a sliding cash scale. Your fee category is determined by your household size and annual income, according to the chart below.

All patients who use the sliding scale are asked to sign an income attestation form, acknowledging that purposefully providing false or misleading information will result in loss of eligibility for the sliding scale rate and an adjustment of fees owed.
