We are currently in-network with Cigna, McClaren Health Plan, Medicare, and United Healthcare. We are in the process of adding more contracts with insurance companies, and our website will continue to be updated in the coming weeks as more are added. Patients’ insurance coverage (expected co-pay/co-insurance amounts and number of authorized visits) can be verified in advance of visit.
We also offer a sliding cash scale. Fee category is determined by household size and annual income. All patients who use our sliding scale sign an income attestation form, acknowledging that purposefully providing false or misleading information will result in loss of eligibility for the sliding scale rate and an adjustment of fees owed.
